What to Expect at Your First Appointment
Your first appointment is a chance for you and your provider to get acquainted. There is no pressure to share everything at once. Your provider will ask questions to better understand your situation, your goals, and what kind of support would be most helpful. Sessions are available in person at our Westminster, MD office and virtually via secure telehealth for clients across Maryland.
Insurance and Payment
We accept most major insurance plans. If you have questions about whether your insurance is accepted, please contact us before your first appointment and our team will verify your coverage. Clients are responsible for copays and deductibles at the time of service.
Confidentiality
All information shared during your sessions and throughout the intake process is kept strictly confidential in accordance with HIPAA guidelines. Your privacy is our priority.
Patient Intake
To complete your intake, please download and fill out the forms below, then upload them using the secure form on this page. Our staff will carefully review your submission before confirming your first appointment. Your information will remain secure and confidential throughout the process.
Download and complete the Service Request Form and any additional forms requested by your provider, such as the MDQ or Vanderbilt ADHD Assessment. Complete the forms electronically or print and complete them by hand. Be sure to sign where required. If completing electronically, save the forms as PDF files with your last name as part of the filename, for example Smith-intake-packet.pdf. If completing by hand, scan or photograph each page clearly. Then use the secure upload tool below to send your completed forms along with any supporting documents such as a copy of your insurance card. Accepted file formats: PDF, JPG, or PNG.
Once your forms are submitted, you will receive a confirmation email. Our staff will review your information and contact you with next steps.
